Letters and Certificate Requests
Thank you for allowing us to serve you. Please fill out the form below so that we can process your request quickly.
Our office can provide a Letter for Event Program or Journal, a Certificate of Recognition, a Personal Certificate (birthday, wedding/anniversary, or retirement), an Adjourning Motion or In Tribute, or a Certificate for another occasion.
Please plan ahead. We ask for a minimum of 3 weeks advance notice to process your request. We appreciate longer lead times. If you have an urgent request, we will do our best to accommodate your deadline, though this may not always be possible. Be certain to call City Hall at 213-473-7008 if you have a short deadline.
Please be aware that certificates cannot be mailed, but must be picked up at City Hall:
